Common Questions

How early should I arrive for my appointment?

A new patient should plan to arrive 20 minutes before their first appointment time to complete new patient paperwork.

Returning patients should plan to arrive 10 minutes before their appointment to check-in. 

 

What information should I bring for my appointment?

Please bring the following documents with you for your appointments:

  • Current government issued photo ID.

  • Social Security Card

  • Insurance Provider Card(s) and/or Mo HealthNet ID Card(s)

  • A list (or the bottles) of all current medications

 

Please arrive at least 20 minutes early before your first visit to complete patient paperwork.

Can anyone use Express Medical Care and Dental Express Care?

Everyone is welcome to our Medical Express Care and Dental Express Care location at 440 E. Tampa St. in Springfield. Patients should use the East Entrance of the building located near the bridge.  

​Express Medical Care

Monday – Friday: 7 a.m. – 7 p.m.

Saturday – Sunday: 8 a.m – 4 p.m.

Dental Express Care

Monday – Friday:  Starting at 7 a.m. until full capacity is reached.

Saturday: Starting at 8 a.m. until full capacity is reached.

Sunday: Closed

Who can help me find other resources?

Our Patient Advocates are able to connect you with our Care Coordination team, Monday – Friday, 8 a.m. to 5 pm. to help patients in all of our locations find resources including housing, food bank and more. Care Coordinators work with our patients’ health care team to connect patients to the best resources available. 

What’s the best way to talk with my provider between appointments?

Send a question with your provider by using our patient portal called NextMD.  Sign up for NextMD here to start communicating with your health care team.

You can also call Jordan Valley at (417) 831-0150 to leave a message with the nursing team that helps coordinate your care. Nursing staff will consult with your provider and quickly return your call.

Does Jordan Valley offer a Sliding Fee Program?

Our Sliding Fee Program is available in all of our clinic locations. Ask one of our Patient Advocates to make a slide appointment to find out if you qualify.
