COVID-19 Information

Jordan Valley Community Health Center is working with community leaders and health organizations to stop the spread of COVID-19. We are a vaccination and testing site.

COVID-19 Vaccines

Vaccines We Offer:
Pfizer (three doses) – approved for ages 5+
Johnson & Johnson (two dose) – approved for ages 18+
Moderna (three doses) – approved for ages 18+
 
Pediatric Pfizer:
Everyone 5+ years who is fully vaccinated against COVID-19 is eligible for a booster. 
  • Booster– Individuals 5 through 11 years of age who completed their 2-dose primary series. The booster dose must be at least 5 months after the 2nd dose of pediatric Pfizer.
  • 3rd Dose– Immunocompromised Individuals 5 through 11 years of age who completed their 2-dose primary series. Must be at least 28 days after the second dose of pediatric Pfizer. Must fill out attached immunocompromised consent form.
 
We are now offering a 3rd dose of the Pfizer or Moderna or a 2nd dose of the Johnson & Johnson vaccine for those who meet one or more of the following qualifications:
Everyone 12+ years who is fully vaccinated against COVID-19 is eligible for a booster. 
  • At least 2 months after a 1-dose vaccine, Johnson&Johnson / Janssen. 
  • At least 5 months after a 2-dose vaccine, Pfizer-BioNTech or Moderna.

**Adolescents 5-17 years old will need a parent/guardian present and signature on their vaccine consent form.
