Our team helps connect you to local resources for housing, food, clothing and other needs, removing barriers to a whole and happy life. Meet with a Jordan Valley team member at any of our community health clinics to get started. You do not have to be a Jordan Valley patient to receive these services.

Employment Placement
Our specialists help you find and apply for jobs. We’ll talk to you about your interests and help make arrangements with potential employers.
Food Insecurity
If you are unable to purchase healthy food each week, you don’t have to go to bed hungry or skip meals. Get connected to resources that help you feed your family.
Housing Needs
Everyone deserves a safe and affordable place to live. Let Jordan Valley know if you need help finding housing or are at risk of losing your current housing. We have on-site specialists that can help.
Legal Assistance
Jordan Valley connects patients to legal services. Get help applying for disability benefits or avoiding eviction. Our on-site specialists can be your legal advocates for special education services, child custody and domestic violence situations.
Medicaid Applications
Medicaid is free or low-cost health coverage. People who cannot pay for medical care may apply for Medicaid. We help with your Missouri Medicaid application and guide you through the process. See if you qualify.
Transportation
Need help getting to Jordan Valley? We help connect you with transportation services.
WIC Program
Pregnant women, breastfeeding mothers and women with toddlers in Springfield, MO, can get nutritious food and healthcare through the Women, Infants and Children (WIC) Program. This free federal program helps moms and their families get fruits, vegetables, diary foods and infant formula.
The local WIC Program is through the Springfield-Greene County Health Department. Contact WIC at (417) 864-1540.
Medication Assistance & Cost-Effective Savings on Medications
Jordan Valley can help you cover some or all of the cost of medications.*
Community Medication Access Program (CMAP)
CMAP provides some free medications. Jordan Valley patients must be uninsured and 18 years of age or older. You must also meet CMAP income guidelines.
Patient's Assistance Program (PAP)
PAP provides some free medications and co-pay saving cards for those with private insurance. You must meet PAP guidelines.

Jordan Valley patients can get discounts at some local pharmacies. The medication must be prescribed by a Jordan Valley provider.

How to Access Our Resources
There are several ways you can receive community services. Once you connect with Jordan Valley, we will direct you to the office or department you need. You do not have to be an established patient to receive any of our community services.
