Jordan Valley maintains a 24-hour answering service. We receive phone calls even when our clinics aren’t open. Our team will return messages quickly to get you the answers you need.

Medical Records Department
Providers and patients can contact our medical records department for release of patient records. Provider requests may be faxed to us directly. Patient requests can be made within NextMD.
Many billing questions can be answered by checking your patient portal, NextMD. If you’ve already registered with our patient portal, please log in.
Requesting medical records for a Jordan Valley patient? Fax the signed Release of Information form to our medical records department. Requests for release of records are completed in the order received.

Requesting Your Medical Records
Requests for records released directly to a patient are fulfilled within 30 days. If Jordan Valley is unable to meet this time period, we will inform you in writing of the reasons for delay and provide a date by which we will complete the request.
Medical Record Fees
Jordan Valley charges a fee of $6.50 for all completed records requests.
