How Can I Express Interest in Services for My Child
We love partnering with local schools to provide quality, accessible care to students across the Ozarks.
If you are interested in obtaining vision, dental, or medical care for your child in one of our mobile units, please fill out this form. By submitting this form, you agree to be contacted by a Jordan Valley Community Health Worker. This Community Health Worker will work with you and your child’s school district to set up care.

Primary Care for School Staff
You can receive routine checkups and services for minor illnesses or injuries. Our mobile health services provide:
- Immunizations
- Physical exams
- Rapid strep and influenza tests
- Referrals to primary care physicians and specialists
Dental
We have dental mobile health clinic units that visit communities and schools to provide care for adults and children. Our mobile services provide:
- Cleanings
- Extractions
- Fillings
- Screenings
- Sealants
- X-rays
Pediatrics
(JV-4-Kids)
We make getting your child to the doctor easier. Our mobile units visit schools during school hours, so you don’t have to take off work. Our mobile services provide:
- Developmental screenings / well-child checks
- Immunizations
- Physical exams and sports physicals
- Rapid strep and influenza tests
School-Based Clinics
Our school-based clinics offer comprehensive primary care services for children, including medical, dental, vision, telehealth, therapy and behavioral health services. These community health services are currently offered in Lebanon, Republic and Hollister schools.
Vision
Our mobile services provide eye exams and can fit you for glasses if needed. If students require glasses, we will deliver them to the school after our visit.

Meeting Patients Where They Are
We provide easier access to care in rural areas. Not all of our patients can come to us. It’s difficult to see a doctor when you have to take off work or travel to another city. We saw a need to bring services to our patients, especially to children who may miss important checkups.
In 2004, a nurse at Jordan Valley proposed we offer mobile health clinic services. She took the first group of staff members to a local school and the rest is history.
