School-Based Care

Our goal is to make accessing healthcare for your child easy. Jordan Valley provides care for students at Hollister schools. Our providers visit for preventative care including medical, dental and vision services.

When your child is sick, the Community Health Worker in your school can arrange a Virtual Visit with a Jordan Valley provider using a TytoCare exam kit. When you pick up your sick child, they will have a diagnosis and, if necessary, a prescription waiting for pickup at the pharmacy of your choice.

Fill out our consent form to ensure your child can access these services.
