A New Online Patient Experience is Coming!

This summer, Jordan Valley Community Health Center will introduce Epic, a new electronic health record (EHR) which gives you access to MyChart.

MyChart allows you to take control of your health information for yourself and your family members.

MyChart is easy to use! You will be able to:

Communicate with your provider – Send a message to your medical provider from the comfort of your own home.

Request prescription refills – Send a refill request for any of your refillable medications.

Access your test results – View your results when available.

View your appointments – View details of your past and upcoming appointments.

When will Jordan Valley patients be able to sign up for MyChart?
Jordan Valley Community Health Center is currently planning to make the transition to our new electronic health record (EHR) system on July 9, 2024. After that transition takes place, patients will be able to sign up for a MyChart account online and access this new patient portal. 
There will be several different ways to sign up for the new patient portal. Be sure to bookmark this page on your browser as you’ll be able to return to it on our website after the transition for a link to sign up for MyChart. Once signed up, you’ll also be able to login online from this page, as well. You may also be able to use a MyChart phone app specifically for Jordan Valley Community Health Center. 
What can patients do now to help the MyChart registration process go smoothly?

If you are visiting Jordan Valley prior to the EHR transition, when you check in, please make sure we have your most up-to-date and complete information. Once the transition is made and MyChart is available to patients, our staff will also be on hand throughout the following weeks to help you with the registration process if you need assistance. 

Why is Jordan Valley transitioning to the new MyChart patient portal?

We believe this will help Jordan Valley improve both patient experience and health. The MyChart portal allows individuals to access their health information and connect with providers more easily. This should prove beneficial to both healthcare professionals and patients.
