Children’s Health Assessments

Jordan Valley Family Development & Early Childhood Education Center includes comprehensive health care for your child(ren), including medical, dental, vision, and behavioral health services.

In addition to childcare and healthcare, we will also complete a development assessment for your child(ren), upon enrollment and prior to moving up to the next classroom.

As a part of the Jordan Valley Family Development & Early Childhood Education Center, your family will receive a needs assessment. This assessment will allow us to identify the areas of needs for your family. Once identified our trained Community Health Workers will help to make these connections including any medical, vision or dental appointments.

Fill out the consent form below to ensure your child can access these services.

Meet the Manager

Kailey Kincheloe has been with Jordan Valley since March 2021. During her time here, she has taken on four different roles and learned from amazing mentors. She is passionate about helping the community, especially its children.

In her free time, Kailey enjoys learning new skills. She likes solving puzzles, crocheting, embroidering, and reading.

“When I started in leadership, my main goal was to create a positive working environment—a place where people are excited to come to work each day,” Kailey shares. “This positivity benefits not only our team but also our patients, as quality patient care stems from happy, engaged workers. Our patients will always be our top priority.”

Development screenings happen at enrollment and again before each classroom move. Community Health Workers are on hand to help connect your family to whatever else you might need.

When you enroll, the center does a short assessment to get a better picture of what your family might need support with. From there, Community Health Workers help make connections to health care, social services, or other resources.

The center is at the Robert W. Plaster Center for Student Success, 2020 Evergreen Parkway, Suite 2, Lebanon, MO 65536. It is open Monday through Friday, 6:30 a.m. to 6:00 p.m.
