Privacy-Related Event

As part of Jordan Valley Community Health Center’s continued commitment to privacy of health information and Health Insurance Portability and Accountability Act compliance, the organization has recently informed patients of a privacy event. Limited information was viewed and taken only for certain patients. The patients whose information was viewed and taken were sent a letter from Jordan Valley.

  • Order a free annual credit report by visiting www.annualcreditreport.com, By calling toll-free at (877) 322-8228, or by completing the Annual Credit Report Request Form on the U.S. Federal Trade Commission’s (“FTC”) website at www.ftc.gov and mail it to the Annual Credit Report Request Service, P.O.
