Make An Appointment
To schedule an appointment, please call (417) 831-0150 and a patient advocate would be happy to help you. You can also just walk in to any of our clinic locations. While there might be a wait, we do our absolute best to see you on the same day you arrive.
Arrive Early to Your Visit
Plan to arrive at least 20 minutes before your first appointment. You will need time to complete paperwork. When you walk through our doors, our team will direct you to the correct waiting area and help you check in. Returning patients should arrive 10 to 15 minutes early to check in.

Needed Documents and Items
Bring these items and documents with you.
- 1 Current government issued photo ID
- 2 Social Security card
- 3 Insurance provider cards and/or MO HealthNet ID cards
- 4A list of all current medications (or the bottles)

Complete Patient Paperwork
When you arrive at our office, you will be asked to fill out paperwork. You can print and complete this paperwork in advance or you may fill it out when you arrive.
