All About Jordan Valley 
 

Jordan Valley Community Health Center is a Federally Qualified Health Center founded in 2003 to provide medical services to the Springfield community. Today, Jordan Valley has locations in six Southwest Missouri communities with nine clinics offering medical, dental, behavioral health and optometry services. Jordan Valley is recognized for its unique integrated model of healthcare with programming focused on bettering our patients and communities one individual at a time.

Our mission to improve our community’s health through access and relationships serves as the nature of all the services we offer. We serve patients through an integrated model of health care, allowing us to meet all of their health care needs in one place. At Jordan Valley we offer medical, dental, vision, and integrate behavioral health care into each service line. As an established Jordan Valley patient they all have the opportunity to meet with a Care Coordinator or a Community Health Worker to ensure they have their other needs met such as transportation, housing, and anything else that may hinder them from receiving health care.

Jordan Valley’s philosophy is “the head is connected to the body.”  We understand that mental diagnoses can impact a patient’s physical body and vice versa.  Behavioral Health is a crucial component of our program providing crisis intervention, substance abuse therapy and compliance therapy.

As part of the Jordan Valley Community Health Center’s mission, we provide patients with access to behavioral health services.

Behavioral Health Consultants are part of the Jordan Valley team, seeing patients in Women’s Health, Pediatrics, Dental, Optical, Express Care and Family Medicine, once a patient has established care with a Jordan Valley provider.

Our Behavioral Health Consultants are the gatekeepers of Behavioral Health services.  Traditional Behavioral Health services include treatment with therapists, psychologists and psychiatric consultation.  Our Behavioral Health consult model allows for:

  • A functional assessment, given by a Behavioral Health Consultant

  • A clinically appropriate plan of action including immediate and brief intervention for mental health diagnoses and behavioral health changes

  • Referral to a psychologist/therapist

  • Assessment for psychiatry

  • Referral to specialized community services

  • Referral to JV Substance Use Clinic

  • Referral to JV Pain Clinic
