Jordan Valley Community Health Center values the privacy and security of our patients’ information. We are providing this notice in accordance with the Health Insurance Portability and Accountability Act (HIPAA) regarding a thirdparty breach involving our health insurance clearinghouse vendor, TriZetto.
What Happened
On October 2, 2025, TriZetto, Jordan Valley’s thirdparty health insurance clearinghouse, discovered suspicious activity within their web portal. TriZetto has confirmed that information was accessed, though at this time they cannot confirm that the information has been misused.
What Information Was Involved
The information associated with the breach varied by individual but could include:
- Name
- Address
- Date of birth
- Social Security number
- Health insurance member number
- Other demographic information
No payment card, bank account, or other financial information was affected.
What They Are Doing
- TriZetto has secured its systems and continues to investigate the incident.
- Notified law enforcement and is cooperating with their investigation
- Implemented additional security protocols to enhance security
- Written notification has been sent to all patients or dependents whose information may have been affected.
- Jordan Valley continues to monitor the situation closely and is working with TriZetto to ensure additional safeguards are in place.
