Jordan Valley only offers services within the United States, and the Website is only intended to be used by users in the United States. Our Service Providers. COPPA Compliance 
The Website is not directed at children under the age of 13. We comply with the Children’s Online Privacy Protection Act (“COPPA”) and do not knowingly permit registration or submission of personally identifiable information by anyone under 13 years of age. If you believe information of a child under 13 has been submitted, please advise us in writing and we will delete it. 

SMS (Text Messages) 
When you sign up for text messages from Jordan Valley through your Electronic Health Record (Epic), you are signing up to receive text messages related to your relationship with Jordan Valley, including updates related to your visits, MyChart patient portal account, one-time passcode, billing notifications, prescription reminders, and care management. 

You can opt out of SMS messages by texting STOP to the respective shortcode. Your opt-out request will generate one final message confirming that you have been unsubscribed. You will no longer receive SMS messages from the shortcode you opted out from. If you want to join again, sign up using MyChart or requesting subscription at any front desk. 

Carriers are not liable for delayed or undelivered messages. 

We will not share your personal information gathered from SMS campaigns with a third party for a third party’s direct marketing purposes without your written authorization. All of the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties. 

Message and data rates may apply for any messages sent to you from us and to us from you. Message frequency may vary.

If you believe your privacy rights have been violated, you may file a written complaint with the JVCHC Privacy Officer at PO Box 5681, Springfield, MO 65801, or by completing a Feedback Form located on the Jordan Valley website, www.jordanvalley.org. According to the law, you will not be retaliated against nor intimidated for filing a complaint with any Jordan Valley entity or the U.S. Department of Health and Human Services.
