How to Become a Patient
Our Mobile Unit program is known for partnering with schools to make sure kids get the care they need without missing school. This also helps parents who aren’t able to take time off work.
Becoming a new patient on one of our Mobile Units starts with just four steps:
- 1 Complete the paper New Patient Forms sent home with your child or these digital forms.
- 2 Mark the day the Mobile Unit will visit your child's school on your calendar.
- 3We see your child on our Mobile Unit during their school day.
- 4 Receive your child's take home form and schedule their next visit with Jordan Valley.

Primary Care for School Staff
You can receive routine checkups and services for minor illnesses or injuries. Our mobile health services provide:
- Immunizations

- Rapid strep and influenza tests
- Referrals to primary care physicians and specialists

We have dental mobile health clinic units that visit communities and schools to provide care for adults and children. Our mobile services provide:
- Extractions
- Full Comprehensive Exam
- Oral Surgery Suites
- Pediatric Consults
- Screenings
- Sealants
- Transportation Assistance
- X-rays
Pediatrics
We make getting your child to the doctor easier. Our mobile units visit schools during school hours, so you don’t have to take off work. Our mobile services provide:
- Developmental screenings / well-child checks
- Immunizations
- Physical exams and sports physicals
- Rapid strep and influenza tests

Our school-based clinics offer comprehensive primary care services for children, including medical, dental, vision, telehealth, therapy and behavioral health services. These community health services are currently offered in Lebanon, Republic and Hollister schools.

Our mobile services provide eye exams and can fit you for glasses if needed. If students require glasses, we will deliver them to the school after our visit.

Meeting Patients Where They Are
We provide easier access to care in rural areas. Not all of our patients can come to us. It’s difficult to see a doctor when you have to take off work or travel to another city. We saw a need to bring services to our patients, especially to children who may miss important checkups.
In 2004, a nurse at Jordan Valley proposed we offer mobile health clinic services. She took the first group of staff members to a local school and the rest is history.
