Welcome to Evergreen
Jordan Valley Health first came to Lebanon in 2015, opening its first clinic to bring medical and dental care to the community. As the need for care grew, Jordan Valley expanded its presence in Lebanon. In 2023, the organization opened its second Lebanon location at 2020 Evergreen Parkway, inside the Robert W. Plaster Center for Student Success.
The Evergreen Clinic was created with families in mind. Today, the clinic offers medical, dental, behavioral health, pediatric, women’s health, and substance use disorder treatment, along with occupational, physical, and speech therapy. Patients can also access same-day sick visits and walk-in care, making it easier to get care without waiting for a scheduled appointment.
What makes Evergreen especially unique is what happens beyond the clinic itself. The location is also home to the Jordan Valley Early Childhood Academy, which serves young children and their families. Children enrolled in the Academy can access Jordan Valley Health services, including items like occupational and speech therapy, during the school day. With no travel time to and from health appointments, students' time in the classroom is maximized—while parents do not need to take off of work for these appointments.
Jordan Valley Health also partners with schools in Lebanon to bring medical, dental, and vision care directly to students through our Mobile Unit program. Through this program, students can connect with Jordan Valley providers while they're at school, helping families get answers and care without always needing to leave work or school for an appointment.
Today, the Evergreen Clinic represents a different way of thinking about community care. Early childhood education, healthcare, and school-based services come together in one place to help children and families get support where they already live, learn, and grow.
