Billing
Many billing questions can be answered by checking your patient portal, NextMD.

Medical Records
Requesting medical records for a Jordan Valley patient? Fax the signed Release of Information form to our medical records department at (417) 831-0155. Requests for release of records are completed in the order received.
Patients can send a request directly through their patient portal, NextMD. If you’ve not yet signed up for NextMD, click here.
Requests for records released directly to a patient will be fulfilled within 30 days whenever possible. If Jordan Valley is unable to meet this time period, the patient will be informed in writing of the reasons for delay and provided a date by which Jordan Valley will complete the request.
Please note, JVCHC charges a fee of $6.50 for all completed records requests. The following records are provided free of charge:
· Continuity of Care (when provided to directly to
another provider)
· OB Travel Papers
· Immunizations, Medications List, and/or Chart
