You don’t need health insurance to get affordable care at Jordan Valley Health. If you qualify for the Slide Discount Program, we will provide lower rates for certain services. Walk in to a Jordan Valley Health clinic to meet with a CHW, and complete your Slide Discount Program application.
Having trouble affording your prescription medication? If you see a Jordan Valley Health provider, you may qualify for an additional discount at any of our in-house pharmacies.Co-pay assistance is also available. Ask your provider to send any prescribed medicines to a JVH in-house pharmacy to get lower prices (thanks to our 340B status) and co-pay assistance. Because being able to afford your medications is part of making healthcare accessible.

Photo ID
Bring a valid ID for the Head of Household, such as a:
• Driver's License
• Non-Driver's License
• State or Military ID Card
• Passport
Proof of Income
The following are examples of items you can bring as Proof of Income:
• Wages/Salaries/Tips/1099 Tax Form
• Unemployment/Worker's Compensation
• Social Security/SSI
• Pension/Retirement Account
• Regular Contributions from Persons Not Living in the Household
• Alimony Received
• Any Other Income Included in Tax Return
Deductions
Bring items that could be deducted on a federal tax return, such as:
• Wages/Salaries/Tips/1099 Tax Form
• Unemployment/Worker's Compensation
