• Jordan Valley Health will be focusing on helping students be ready for school from July 13, 2026-September 30, 2026. These services will also be available before and after this time period.

• Hollister
• Republic
• Springfield Grand
• Springfield Tampa
• Lebanon Highway 5 (North)
• Lebanon Evergreen (South)

• A Jordan Valley Health mobile unit / with a Jordan Valley Health school-based care program 

• At a Jordan Valley Health Start Strong Back-to-School event  

 
You do NOT have to wait until a Start Strong Back-to-School event to see a provider for back-to-school screenings and/or immunizations. 

A care team member will work with a student entering Kindergarten through Grade 12 for getting preventive care for 2 or more of the following services: 

• Well-Child Check-Up (within the last 12 months) 

• Vision Exam (within the last 12 months) 
  OR complete a vision screening at the event  

• Dental Exam (within the last 6 months) 
  OR complete a dental screening at the event  

• Immunizations (up to date) 
  OR receive immunizations at the event  

• Sports Physical (within the last two years) 
  OR complete one at the event 
  
  

These services listed are based on recommended health screening guidelines for students. 

Yes! The events themselves are no cost to attend.  
 
For example, if you decide to attend the back-to-school event at our Grand Clinic in Springfield, there is no event admission or cost to come and get a balloon animal, play games, and/or grab a snack. 
 
There may be a fee for some health services.

We are committed to help make services more affordable and connecting you with care. You may be eligible for assistance. Jordan Valley Health offers a Slide Discount Program based on household size and income for uninsured patients or for services that are not fully covered by insurance. We also accept Medicaid. If you are concerned about your ability to pay for care, please call us at (417)831-0150 (or walk in) and ask to speak to a Community Health Worker.
