Informed Consent for Focus Group Participants

You are invited to participate in a focus group discussion conducted by Jordan Valley Health. The purpose of this session is to gather insights and your perceptions of Jordan Valley Health, health services available in the Ozarks, and the healthcare industry. Your feedback will help us improve our services and enhance patient care.

Your participation in this focus group is completely voluntary. You may choose to withdraw at any time without any consequences to your medical care or relationship with our facility.

While we will make every effort to maintain confidentiality, please be aware that focus groups involve multiple participants, and absolute privacy cannot be guaranteed. We request that all attendees respect the confidentiality of others and refrain from sharing any information discussed outside the session.

This focus group may be audio and/or video recorded for internal evaluation. The recordings will be securely stored and used solely for improvement purposes. If you do not wish to be recorded, please inform the facilitator before the session begins. Additionally, your participation may involve sharing health-related experiences. If any Protected Health Information (PHI) is discussed, it will be handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA). By participating in the focus group, you explicitly permit that Jordan Valley Health may record, store, or associate any identifiable health information with your responses.

There are no direct risks associated with participating in this focus group. However, discussing personal or medical experiences may cause emotional discomfort. You have the right to refrain from answering any questions that make you uncomfortable.

Participants will receive compensation of $25 for their time and contributions to this focus group. Compensation will be provided at the conclusion of the session via check. If you choose to withdraw before the session is completed, compensation will not be provided.
