The Day Center

In 2024, Jordan Valley Health opened a new chapter in how we care for older adults in southwest Missouri. With the launch of Jordan Valley Senior Care (JVSC), we brought the region its first Program of All-Inclusive Care for the Elderly (PACE), creating a new way for eligible seniors to receive the care and support they need while continuing to live at home.

PACE is built around a simple idea: growing older shouldn’t mean giving up the comfort of home or the connections that make life meaningful. Our work enables senior adults (and children of aging parents) to feel safe at home while avoiding traditional nursing homes. Through Jordan Valley Senior Care, participants receive a personalized care plan and support from a team that works together to meet their medical, personal, and social needs. This includes everything from primary care, medications, and therapy to transportation and in-home help.

At the heart of the program is our Day Center at 1930 W Grand St. It’s a place where participants can access care and therapy, share meals, take part in activities, and spend time with others. Our team also provides in-home support, helping participants stay safe, healthy, and connected to their community.

As the first PACE program in southwest Missouri, JVSC is helping bring a new approach to aging and healthcare to our region. We’re proud to support older adults and their families with coordinated care that puts each person’s needs, independence, and quality of life first.
