What to Expect on
Your First Visit
-
Check In
When you arrive, a Patient Advocate will greet you and help you with check-in and any required forms. Be sure to bring your photo ID, insurance card*, and a list of any medications you take.
*If you don’t have insurance, we’ll talk with you about programs you may be eligible for, including programs like a Slide Discount Program or Medicaid. We’ll work through any barriers together to make the next step feel possible.
-
Meet Your Care Team
You’ll meet your provider and care team. They will listen to your concerns, answer questions, and work with you to create a care plan tailored to your needs. This may include connecting you with a Community Health Worker (CHW) for support with housing, employment, food, medications, prescription co-pay assistance, and/or transportation to and from appointments, as needed.
-
Talk About Your Health
We’ll ask about your health history, past care, and goals. This helps us understand the full picture so we can provide the best care for you as your health home. We provide a wide variety of services for the whole family, so every visit becomes a turning point to continue improving your health.
-
Download MyChart
Your Care Team will help you download MyChart if you are not already signed up.
We want to be sure you're connected with your Care Team to stay in communication about your next steps, including follow-up visits, lab work, or additional services. This is also how you will view any test results and message your provider about questions or to request a prescription refill.
-
Check Out
Before you leave, be sure to check out and also book your follow-up appointment(s) (if recommended by your provider).
