Q: Does the COVID-19 vaccine cost anything?

A: No. The state is providing the vaccine at no cost. Some organizations might charge an administration fee to their insurance, but we are not currently charging a fee.

Q: Do I need an appointment to receive a COVID-19 vaccine?

A: Yes. At this time we are sending out scheduling information upon vaccine availability. Please fill out this form to be contacted when we have opening appointments.

Q: Do I have to have my consent form with me when I go to my vaccine appointment?

A: It is preferred that patients arriving to their vaccine appointment have a completed consent form with them. However if you are not able to print one we will have one available for you at check-in.

Q: Will I get my second dose with Jordan Valley as well?

A:Yes. You will receive an email to schedule your second dose. You should get your second dose 21 days after your first dose.

Q: Do you throw vaccine away if it is not used at an event?

A:No. We do not throw vaccine away. We can get 5-6 doses out of a vial of vaccine. As we get close to the end of an event, we wait for 5 people to arrive before we prepare doses so we do not waste any.
